Advising Management: the Recommendation and the Paper Trail

Unit 2: Procurement, Contracts, and the Asset Lifecycle. Lesson 12 of 60 in CHTM Academy.

A surgeon has already chosen a system, the vendor has already quoted it, and the capital committee meets in nine days. You are copied on the email as a courtesy. This is the most common and the most expensive way HTM is used in procurement, and the whole lesson is about how to be in the conversation three months earlier instead.

HTM's actual role

Consult early, evaluate, recommend. Do not decide alone. The clinical department owns the clinical need, finance owns the money, and leadership owns the decision. What HTM owns is the technical truth: whether the hospital can run, service, secure, and afford this device for its whole life. Being in the planning and design meetings is not a courtesy, it is where equipment decisions become construction costs and service costs nobody budgeted.

The evaluation record

Element What it holds Who supplies it
Clinical input the need, the volume, the the clinical director,
consequence of not funding in writing
Technical review serviceability, parts, manuals, HTM
                       test equipment, training, space,
                       power, and site preparation
Cybersecurity review the MDS2, the operating system HTM with IT and the
and how long it is supported, security officer
                       patching, authentication,
                       encryption, remote access
Service strategy in-house, contract, or time and HTM
                       materials, priced for the life
Total cost of purchase, installation, service, HTM with finance
ownership parts, training, software,
                       consumables, disposal
Standardization what the hospital already owns HTM
and what a new model adds
References other facilities running it, and HTM, directly, not
what MAUDE already holds through the vendor
Options compared every option including doing HTM
                       nothing, over the same years

Conflicts of interest and vendor relations

Vendor relationships are useful and must be visible. Disclose anything of value offered to you or your staff under the hospital's policy, decline what the policy forbids, and never let a vendor write your evaluation or supply your references unverified. Treat all bidders the same: the same specification, the same questions, the same deadline, the same information. Keep pricing confidential between bidders. The test is simple: would this look defensible written down in the file, because one day it will be read.

Writing a recommendation leadership can act on

One page, one recommendation. The clinical need in a sentence, the options with their total cost of ownership over a stated number of years, the recommendation, the risks of the recommendation and how each is handled, what happens if nothing is funded, and the decision you are asking for. Put the number and the recommendation in the first three lines; a committee can approve a line and cannot adjudicate an essay. Say what you do not know and when you will know it rather than leaving a gap for someone else to fill with an assumption.

What the file must hold for a later audit

The specification, the quotes and who was asked, the evaluation record above, the conflict disclosures, the committee minutes or the approval, the purchase order, the acceptance test results, and the service strategy as it was approved. An auditor or a surveyor asks the same question in different words: show me how this was decided, and show me that the same rule applied to everyone. The file answers that or it does not.

Exam tip: HTM consults early, evaluates, and recommends; it does not decide alone. The evaluation record carries clinical input, technical review, cybersecurity review, service strategy, total cost of ownership, standardization, and verified references. Disclose conflicts, treat all bidders alike, and keep the file that shows how the decision was made.
Remember
HTM's role in procurement is to consult early, evaluate, and recommend, leaving behind a one-page recommendation and a file that shows an auditor exactly how the decision was made.

Advising Management: the Recommendation and the Paper Trail, explained slowly

The one-page recommendation

This is the whole document. Anything longer is an attachment.

TO Capital Committee
FROM Healthcare Technology Management
SUBJECT Replacement of three anesthesia machines, OR 2, 4 and 6
DECISION Approve Option B, $462,000 capital, in the current fiscal year
REQUESTED

NEED The three machines reach end of manufacturer support in
November. Parts and service training are withdrawn on that
date. Surgical services runs 2,100 cases a year through
these rooms and has no alternative machine on site.

OPTIONS Ten-year total cost of ownership, all three rooms
A Do nothing 0 capital, support ends,
                                       no compliant service strategy
B Replace with the 462,000 capital, 318,000 service
hospital's current and parts, 780,000 total
               standard model
            C Replace with a new 418,000 capital, 455,000 service
manufacturer and parts, 873,000 total, plus
                                       training and test equipment

RECOMMEND Option B. It is $93,000 cheaper over ten years despite a
higher purchase price, it matches six machines already in
service, and staff are already competent on the model.

RISKS Delivery is 16 weeks; order by June to install before
November. Mitigation: order on approval, not on delivery of
            the capital release.
One room requires a gas outlet change. Facilities has
            quoted $11,000, included above.

IF NOT From November the machines run with no manufacturer support,
FUNDED no parts route, and no compliant service strategy. Surgical
services would need to reduce to three rooms on any failure.

Note what is in the first four lines: the decision requested, the money, and the date. A committee can approve that. Note also that Option A exists and is described honestly, because a recommendation with no alternative reads as a demand.

The evaluation record, as a checklist with owners

# Element Owner Evidence in the file
1 Clinical need and volume clinical director signed statement

                                                       writing by the maker
3 Test equipment and tools HTM list and cost

                                                       the record interface

                                security officer support horizon, patch
                                                       method, authentication,
                                                       encryption, remote access
7 Service strategy, priced HTM in-house, contract, or
                                                       time and materials, over
                                                       the life
8 Total cost of ownership HTM with finance the ten-year comparison
9 Standardization HTM what the hospital owns
and what a new model adds
10 References HTM two facilities contacted
                                                       directly, plus what MAUDE
                                                       already holds on the model
11 Conflict disclosures every evaluator signed, dated
12 Options compared HTM including doing nothing

Row 10 has a rule attached: contact the references yourself. A reference list supplied by the vendor and never called is worse than no reference list, because the file now implies a check that did not happen.

Conflicts of interest, in practice

Situation What a defensible manager does
A vendor offers to fly you to a user check the policy; usually permissible
meeting during an active evaluation only if the hospital pays, or not at all
                                            during an evaluation
A vendor offers lunch for the shop disclose it; follow the policy's value
                                            threshold
A vendor asks what the other bid was decline; pricing stays confidential
                                            between bidders
A vendor offers to write the technical decline; they may answer your
specification questions, they may not set them
A relative works for a bidder disclose in writing and recuse yourself
                                            from scoring
A vendor's representative is a friend disclose, document, and keep every
                                            communication inside the process

The test is the file: would this look defensible written down, because one day it will be read by somebody who was not there.

What the file holds for an audit

Specification and the requirement it came from
Who was asked to quote, and the quotes received
The evaluation record above, complete, with dates
Conflict disclosures, signed
The recommendation as submitted
The committee minutes or written approval
The purchase order and any sole-source justification
Acceptance test results and the incoming inspection
The approved service strategy and the contract, if one was signed
Training delivered and competency recorded

What to memorize

HTM's role consult early, evaluate, recommend; do not decide alone
Evaluation record clinical input, technical review, cybersecurity review,
service strategy, total cost of ownership, standardization,
                  verified references
Conflicts disclose, follow policy, treat all bidders identically,
                  keep pricing confidential
Recommendation one page, one recommendation, decision and money in the
first lines, and what happens if nothing is funded
The file shows how the decision was made and that the same rule
                  applied to everyone
